POSITION SUMMARY:

  • The Nurse Practitioner (NP) brings expert care to members, through clinical assessments, screenings, and education. This role may provide care in a variety of settings including member/participant homes, assisted living facilities, Telehealth visits, and skilled nursing facilities.

ESSENTIAL JOB FUNCTIONS

  • Perform screenings including medical history, physical exam, medication reconciliation, and education.
  • Effectively identify, evaluate, and address the concerns of the member, employee, or participant.
  • Administer and order point-of-care and lab-based testing.
  • Order and interpret diagnostic imaging, and laboratory results and provide clinically appropriate interventions, additional tests, or referrals as needed.
  • Refer members to PCP, case management, disease management programs, and/or appropriate specialist as necessary.
  • Maintains positive control of all PHI information and compliance with OSHA and the State Board of Nursing.

CREDENTIALS:

  • Active Nurse Practitioner License in the State of Maine
  • Board Certification by AANP, ANCC or AACCN
  • Minimum of 1 year of experience as a Nurse Practitioner

MUST HAVE:

  • Must be capable of traveling to & from each patient's home
  • Valid state driver's license and proof of coverage
  • Must be Covid Vaccinated
