Nurse Practitioner (full-time, PRN, and travel opportunities)
POSITION SUMMARY:
- The Nurse Practitioner (NP) brings expert care to members, through clinical assessments, screenings, and education. This role may provide care in a variety of settings including member/participant homes, assisted living facilities, Telehealth visits, and skilled nursing facilities.
ESSENTIAL JOB FUNCTIONS
- Perform screenings including medical history, physical exam, medication reconciliation, and education.
- Effectively identify, evaluate, and address the concerns of the member, employee, or participant.
- Administer and order point-of-care and lab-based testing.
- Order and interpret diagnostic imaging, and laboratory results and provide clinically appropriate interventions, additional tests, or referrals as needed.
- Refer members to PCP, case management, disease management programs, and/or appropriate specialist as necessary.
- Maintains positive control of all PHI information and compliance with OSHA and the State Board of Nursing.
CREDENTIALS:
- Active Nurse Practitioner License in the State of Maine
- Board Certification by AANP, ANCC or AACCN
- Minimum of 1 year of experience as a Nurse Practitioner
MUST HAVE:
- Must be capable of traveling to & from each patient's home
- Valid state driver's license and proof of coverage
- Must be Covid Vaccinated
